This position has NOT been filledSt. Joseph's Mercy Care Service's (SJMCS) mission is “to participate in the healing ministry of the Church and the Sisters of Mercy by improving the well-being of persons who are poor and disenfranchised, through the promotion of health, the provision of health services and advocacy for healthy communities.” At four fixed and seven mobile clinic sites across metro Atlanta, SJMCS provides an array of services including primary and preventive care, health education, oral health care, vision services, mental health case management and recuperative care for medically fragile homeless men. Mercy Care Services offers primary healthcare to the uninsured, economically disadvantaged and medically underserved. The AHC member will be serving as a Patient Navigator, helping to educate and guide our patients as they access medical services both internally and externally.

Patient navigation refers to the assistance offered to healthcare consumers (patients, survivors, families, and caregivers) to help them access and then chart a course through the healthcare system and overcome any barriers to quality care. A patient navigator serves as a "guide." The member will assist patients move through the complexities of the healthcare system—getting them more timely treatment, more information about treatment options and preventative behaviors.
This position has been filledThe DeKalb Community Service Board (CSB) is a nonprofit public safety net provider of community-based behavioral healthcare, offering a full range of mental health services, substance abuse treatment and developmental disabilities programs. Our staff of physicians, nurses, clinicians and support personnel is dedicated to helping out clients and their families recover from these debilitating disabilities and resume productive lives. Many clients using the facility are without financial means. The only way to obtain effective drug therapy is through a drug manufacturer sponsored patient assistance program. The time requirement necessary for the paperwork is a substantial burden on the pharmacy staff. The AHC member will allow many indigent citizens to receive many medications they would otherwise be unable to obtain.
The member will assist clients of the Winn Way Mental Health Center clinic with enrollment in Pharmaceutical Medication Assistance Programs (PAP).
